Face ID or passcode lock. Backup and restore you start. Apple Health import is read-only. iCloud sync is optional. The website does not collect a waitlist.

AllergyVault Plus
The core vault stays on your phone. Plus is there for extras.
Plus covers iCloud sync, a full family vault with caregiver invites, travel language packs, time-limited share links, Apple Watch Medical ID, menu and label scan assist, school and camp templates, and priority support. Emergency ID, kitchen cards, and the on-device vault do not require Plus.
